ABSTRACT 患儿，女，9岁3月龄，因“双手指间关节疼痛1个月余，双下肢肿胀1周”于2024年2月就诊于深圳市儿童医院风湿免疫科，伴发热、蝶形红斑、尿少，辅助检查提示白蛋白、补体C3、补体C4、白细胞、血红蛋白下降，尿素氮、肌酐升高，血尿、蛋白尿阳性，抗核抗体、抗双链DNA抗体、抗SSA抗体等多项自身抗体阳性，肾脏病理提示弥漫球性增生性狼疮性肾炎Ⅳ-G（A）型。诊断系统性红斑狼疮及狼疮性肾炎，予足量醋酸泼尼松联合吗替麦考酚酯、羟氯喹治疗，2周后患儿关节疼痛好转，发热、浮肿、皮疹消失。出院后二次全外显子基因检测回报显示TNFAIP3基因c.259C>T自发变异导致第87号氨基酸由精氨酸变为终止子，增加病因诊断A20单倍剂量不足。随访6个月，患儿无明显疾病活动。.
